The Luksenburg System for SUI and Tissue Laxity: Replacing TVT/TOT?
Now cleared in the USA for mild to severe SUI and Labia Majora Laxity. The goal to create a new and firmer mid urethral floor using PCL Threads and PRP. In-office, awake, local anesthesia.
Ariel Luksenburg, M.D., a gynecologist from Uruguay, introduced his system for the treatment of severe stress incontinence to the U.S. in 2021 at Marco Pelosi’s International Society for Cosmetogynecology Pre-Congress workshop in Fort Lauderdale, FL. Dr. Marco Pelosi’s (II and III) cadaver lab centered on female genital aesthetic surgery. Dr. Luksenburg demonstrated his technique to dozens of surgeons from around the world. The course was repeated in 2022 and most recently in March of 2023. This time with FDA clearance to use his threads and patented delivery system. Each year my fascination grew regarding the Luksenburg System but I was unsure if it was ready for Prime Time use in the USA. After hearing the system patent was granted, the PCL threads becoming CE and FDA compliant, and seeing the 3 year data presented, I became a believer. I was flabbergasted and honored when Ariel Luksenburg and Jorge Gaviria invited me to become the first surgeon in the U.S. to be trained, perform, and to teach The Luksenburg System. They committed to also train my California colleague Judy Wei, MD. I would join several hundred doctors in Latin America who were already trained, certified, and proficient. So we planned, prayed, and got trained on July 24 and 25, 2023, in Irvine, California, at Dr. Judy Wei’s beautiful office.
The video is a primer on the Luksenburg System explaining how it is done. I added the section on labia majora tightening using PCL threads and Dr. Luksenburg’s special PRP potion to tighten and regenerate tissues. I will present Dr. Luksenburg’s presentation and data in more detail on another upcoming newsletter.

So, will the Luksenburg System replace TVT and TOT? It may for many but perhaps not all. It is not covered by insurance so that wipes out alot of customers. Its results are pretty on par with reported success rates for TVT and TOT. It is definitely cheaper as a whole for society when you realize that hospital and anesthesiology costs are eliminated. It is done under local anesthesia in your office and the safety profile is definitely superior to the slings plus you do not use any polypropylene that can erode. The 99% cured/improved results are impressive at 29 months average and Dr. Luksenburg has patients many more years out with enduring results. It looks like it can help just about all stress urinary incontinence patients. Does it last as long? Not sure as we only have about 3 year data.
