Radiosurgery for the Cosmetic Gynecologist: A Brief Tour Through Time

I last covered Radiosurgery 101 for the Cosmetic Surgeon on May 30, 2022. Go see that 15 min video if you have not already done so. This is an update and full length version on what radio surgery can do for labiaplasties, botched surgeries, clitoral hood surgeries, anal skin tags.

The video Below shows specifically Soniquence Radiosurgery in action. Soniquence is the rebirth of Ellman International. I first worked with Ellman in 2005 and taught worldwide to advance radiosurgery. Cynosure acquired them and buried them deep in their closet and it faded in use and memory. Very sad. The original brains behind Ellman then created Soniquence to advanced radiosurgery once more. Soniquence was able to get Ellman back from Cynosure and now they live under one umbrella. Background history on Radiosurgery

I started using a radio surgical device called the Ellman Surgitron with a fine Hair Tip back in 2005. Super precise with lateral thermal spread measured in microns (10-20 microns). I taught many Fellows at Laguna Beach for almost 20 years.

The Ellman Cosmetic Gynecology Course circa 2009

Around 2009 Ellman’s newest upgrade became known as “Pelleve.” This device had the advantage of being able to do non-surgical tightening of tissues. It was meant for face and body but of course I used it on the labia! I even designed a vaginal probe for it. Here is my prototype no one has seen before. It was meant to go inside the vagina:

Ellman made me several of these for my Pelleve. I started trials for vaginal use and had wonderful results to reduce mild to moderate stress incontinence and overactive bladder. With a handshake, Ellman and I decided to go forward and create a whole new business of intravaginal and vulvar radiofrequency treatments in 2009. I continued to perform many treatments with exceptional safely, no burns, no complications. The is the precursor of my ThermiVa patent and device that would be FDA Approved 6 years later in 2015.

This was the science behind Pelleve’s skin tightening in visual form.

This was the one I found best for exceptionally refined labiaplasty and feathering procedure. This little device became quite famous.

Of course I was more well known for labial surgery so I focused on applications for labiaplasty and vaginoplasty and clitoral hood and anal skin tag surgeries. I taught and promoted Ellman’s Next Generation Pelleve:

The Beginning of the End for Ellman

In 2009 Ellman and I had agreed to proceed to create the first intravaginal probe for radiofrequency application. Unfortunately Ellman chose to purchase Sandstone Lasers and it did not go well. Ellman was forced to eventually sell itself to Cynosure, the Big Gorilla at the time. Cynosure had a different culture and relationships developed over decades by Ellman were lost. I stopped working with this version of Ellman. Ellman became a product lost in the vast array of devices that Cynosure managed. Cynosure would then sell itself to another buyer and Ellman was cast off. You could not get service, supplies, disposables. It was very stressful for Ellman users for a couple of years from about 2019 to 2021.

ENTER SONIQUENCE!!!

In 2021 I got a call from Bill Bernstein. He used to run the Ellman division for Ellman International then Cynosure. Little did I know that a new company named Soniquence had been created by Alan Ellman, son of the founder of Ellman International. He brought back the old team and placed Bill as VP! Bill sent me the newest SmoothWave i200 to try. The big selling point was the much improved Turbo Mode on the Bipolar settings that dramatically improved the hemostatic abilities of the device. RF has long been known to be poor at hemostasis and now with the improved Bipolar energies it had overcome those issues! I tried it for several months, did tons of cases, and I am solidly on board! They focused on surgery and stopped trying to be a Swiss Army knife. No more non-surgical portion. I became a Soniquence believer because of their dedication to the surgical arts and their excellent customer support.

I switched to Soniquence and got my device in 2021. I taught many surgeons on its use. I lectured worldwide also without compensation or a professional business relationship with the company because I was a true believer paid or not. It was the best device for labial surgery. Bar none.

I sold my live and onsite teaching program at the end of 2022 and gave one last course in the summer of 2023. Now the young guns could teach the live courses and I would support them.

The Turbo Mode with the Bipolar was an awesome advance in hemostasis.

Starting my lecture circuit on radio surgery

Presenting Soniquence to the world for use in the Management of the Buried Clitoris. Dubai, Istanbul, Peru, to India, to Philippines, to Mexico and Guatemala.

Mike Reed and others took over the training program for Soniquence while I concentrated on placing my surgical videos on Gynflix for worldwide access.

I continued to travel all continents to show my surgeries with RF.

I stressed the exceptional precision and lack of lateral heat damage.

I began developing specific bipolar handpieces for labial surgery.
