How to add Labia Minora when there isn't any left. Our secrets revealed. Finally.

A surgical technique I held close to the vest and shared only with my Alinsod Fellows is finally seeing the light of day. Congratulations Drs. Reham Awwad, Natalie Sorial, and my dear friend Amr Seifeldin for publishing this landmark article on what to do when too much labia minora is excised during labiaplasty surgery. A common problem encountered by new labiaplasty surgeons. This excellent article covers background history, FGM (Female Genital Mutilation), and the advanced surgical methods to manage both cosmetic and functional issues. Dr. Amr Seifeldin is the world’s foremost and most experienced gynecologic surgeon that deals with these life altering problems on a daily basis in Cairo, Egypt. His teaching program reaches from Egypt to Dubai to the Philippines and Brazil, and beyond. Together with Plastic Surgeon Reham Awwad they have refined my Grooving technique and have made it their own. Background

I developed the concept of the Barbie Look Labiaplasty in the 1990s and early 2000s. I formalized it in 2005 and started lecturing and teaching about it at that time. If you are unfamiliar with what a Barbie or Hybrid or Rim Look Labiaplasty are then go to my website for a quickie description: Labiaplasty Reduction. On a very rare occasion (less than 1%) a patient would change her mind and regret having all of her labia excised. There was really no published or known method out there on how to create labia from no labia so I created the Grooving Technique in 2006-2007 using radiofrequency surgery. I really did not create or add labia and it was more of an optical illusion to make it look as if there was a small amount of labia minors left. It became the core procedure of my Revision Labiaplasty Practice. I never published it. About 5-6 years later Dr. Gary Alter published his flap technique used by many plastic surgeons. Dr. Amr Seifeldin came to Laguna Beach for my Fellowship Training in 2015 and 2016 so I showed him my Grooving method and he immediately “Got It!” He applied the concepts I showed him to create larger Labia Minora from Labia Majora tissue and completely transformed the world of labiaplasty surgery. He deserves the credit for expanding on my “Optical Illusion” and making it his own “Labial Creation” surgery. Bravo Amr! Bravo Reham! Bravo Natalie!

The Publication

This is the case reported in the article shown in various angles. Originally she had large and puffy Labia Majora that she felt were “like testicles.” She wanted a labia majoraplasty with fat pad removal and a Barbie Look Labiaplasty (removal of almost all labia minora). Months after surgery her results are shown on the top row. She changed her mind and now wanted a revision to show more labia miniora. I performed the grooving technique and created a trough between the minora and majora and made sure those edges stayed apart. She used a Q-Tip daily to prevent the edges from rejoining together and loosing the visual optical illusion of a new unattached labia minora. The middle row shows the results immediately after Grooving. The bottom row shows the results several months later. She was much happier with this petite and athletic appearance.
