A brief history on how the Cellular Medicine Association (CME) has had a huge influence on Feminine Wellness and Cosmetic Gynecology. And how to get yourself trained.
Many of you know Charles Runels, MD, as the inventor of the O-Shot, P-Shot, Vampire Facial, Vampire Face Lift, Vampire Breast Lift, and now Clitoxin. Charles has been a great friend since we first met in October 2015 at my Congress on Aesthetic Vulvovaginal Surgery Congress (CAVS). Here is a look back at our decade long collaboration written and shown like a LIFE MAGAZINE spread telling a story with pictures. Charles trained me in his procedures and I have continued to spread his knowledge through my avenues.

Charles started his medical career in Primary Care. A son of Alabama, he told me of his days in the Emergency Room, his research, and battles with traditional medicine. He was a man way ahead of his time and one who wanted to carve out his own path. I can relate.
Charles found the science of blood to be fascinating so he jumped in with both feet and read everything there was about Platelet Rich Plasma. Cardiac surgeons and orthopedic surgeons were using this science to help their patients recover quickly because of its regenerative properties. Charles wondered how this could be applied to other areas of the body. What about use in the clitoris, G-Spot, penis? It is your own blood, has been used thousands of times without injury, so why not try to see if it aids in sexual function.
With himself as his own patient and with the help of adventurous others Charles set on a path of discovery. He found PRP to increase blood flow, aid healing, and improve genital sensitivity. He became the most influential non-gynecologist in the world of gynecology and sexual health. And many resented him and were jealous of his accomplishments. He was at the receiving end of lots of arrows. Mostly from bows of gynecologists. And that is what attracted me to Charles and his story.
Around 2013-2014 I started hearing about a doctor in Fairhope, Alabama, who was injecting patients with their own blood into their genitals. I had been developing ThermiVa and radiofrequency for feminine use at this time so my interests were also about increasing the blood flow to the genitals and improving sexual health. I got in contact with Charles and invited him to my 10th and last year of my CAVS Congress and he said “Yes” I would like to come and present my research. October 8, 2015 came and we finally met.
Charles captivated all of us. The CAVS audience was mostly gynecologists, plastic surgeons, and dermatologists. We were all blown away by his passion and convincing talk on the O-Shot and P-Shot. I knew this was just the dawn of a new age. This was the start of biologics for gynecologic uses. My work with radiofrequency was the start of energy use for gynecologic conditions. Together I then believed that there would be Synergy of Energy plus Biologics that would move our Cosmetic Gynecology specialty forward for the next decade.
This was my Faculty for my very last CAVS 2015 in Orlando. All are my dear friends and pioneers in Cosmetic Gynecology. After 10 years of running CAVS I decided to unite with the Pelosis and bring unity to the American Cosmetic Gynecology community.
So in March of 2016 I flew to Fairhope, Alabama. It is a 3 stoplight town I am told. Gorgeous and right on the Gulf Coast. There was Charles in his element surrounded by doctors and nurses yearning to learn this new thing. I was a surgeon so this really intrigued me on how I could use this science to aid healing and get better patient results.
Charles had been giving these course for several year already and more gynecologists were hearing about it and showing up. Lots of nurses, PAs, Primary Care Internists and FPs would attend. The venue was wam and cozy and very charming. It was not trying to be Beverly Hills cool or Manhattan chic. It was more folksy and Southern and personal.
Charles started with his personal history and journey (you must ask him yourself personally and he will tell you!) then went into the science and technical process. Most of us had no clue what PRP was or how it worked. I did not know how it could help my primarily surgical practice. I was about to be blown away.
Charles loved his clean white sheets of paper. He gave a very visual oriented teaching. Lots of graphics and photos too. He would sit on his throne and project his computer screen into the larger screen up front. Then he would hand draw to stress important points.
After drawing the pictures he would tape them up on a wall, bring them here and there, and use it as visual aids as he did manual things. I kept one of these drawings from a past training and will frame it one day. It will be worth a Picasso in the future. Grab one when you have the chance.
Charles would then focus on the mechanics and techniques of obtaining PRP. He showed us the various devices he had at his disposal. He refused to “work” with a company as a paid endorser and used whatever he thought was best. You can rest assured he will give his opinion and not be biased when he gave advice on equipment and devices as he is not in the pocket of companies. He did most of the prep work himself and lined up all the labeled syringes logically.
Charles showed us how to mix PRP with HA with the right proportions for use in the face. Later on he would invent and develop the Vampire Wing Lift that placed larger amounts of both PRP and HA into the Labia Majora. That was the inspiration behind my development of FemXHA for Vulvovaginal Filling. FemXHA would not exist were it not for the influence of Dr. Charles Runels.
Showtime! Let’s inject!
We started with faces and how to do Vampire Face Lifts (PRP + HA) injections and also learned the infamous Vampire Facial for getting that glowing complexion. Everyone got to inject with Charles guiding. There were enough patients for everyone to be able to inject in the face and in the genitals. The sessions were like clockwork and highly efficient. The rotations were made so that you got to go to the front of the line to see and do. That protected the wallflowers.
The next focus of training then became genital. We reviewed of anatomy of the Clitoris, G-Spot, Vulva, Penis. Charles showed us where he injected the clitoris and penis. He provided funny and useful anecdotes. I loved his teaching style. I had my own course in Laguna Beach at the time (started in 2005) so I was impressed with the way he shared fully his knowledge without holding anything back. I started seeing how non-surgical treatments could be useful in enhancing my teaching program back home. I had already added amniotic fluid, radiofrequency, Lasers, Predictive Permeation, and now I wanted to add the O-Shot and Vampire Wing Lift (vulvar plumping)
Dr. Runels spent a great deal of time speaking with you and answering all your questions. All members received his personal mobile number and email and were encouraged to communicate directly with him and bypass the administrative team if the issues were clinical in nature. I have had his number on my speed dial.
Charles would also make sure to educate his patient and make therm feel as comfortable as possible. It could be intimidating for the patient surrounded by practitioners most of whom had never done the procedure before. They were comforted and reassured personally by Dr. Runels.
Each attendee was able to sit right beside Dr. Runels and witness first hand where to inject. The anatomy was reviewed, numbing was done.
When the injections were done every provider was given the chance to be up close see exactly where the injections are placed and how much was given. The patient was never surprised as she was told every step of what was happening and a staff member would be by her side giving verbal anesthesia.
After witnessing first had how the procedure was performed by the Master Injector, each clinician then stepped up and had a chance to inject themselves with direct assist from Dr. Runels and his staff.
The same rotational steps allowed interested providers to see and perform the P-Shot. I was quite amazed how comfortable the procedure was for the patient. Patients also came in for their Vampire Wing Lifts of the Labia Majora and Vampire Breast Lifts for more breast definition. You were able to see and even perform the procedures you were interested in learning for your personal practice. At the end of the first clinical day was a wonderful dinner and fun social gathering that went all night. Day 2 had more cases so that everyone who wanted to could do the injection of their choice. Day 2 also had an intense marketing training session and how to succeed with this type of practice. It was as important as Day 1.
Before and after Charles Runels spoke at my CAVS 2015 meeting he was sought after worldwide. He literally trained the world in the art and science of using PRP for genital use. In my opinion his speaking at a the annual ISCG meetings (International Society of Cosmetogynecology) propelled him to a much deserved status as The-Go-To-Guy for anything biologic and blood related for our emerging specialty of Cosmetic Gynecology. Everyone wanted to listen to Charles and be exposed to his insight. Several programs started around the world that tried to imitate what he had created. Many plays on the terminology “O-Shot” and “P-Shot” and “Vampire” trademarked procedures arose and many modified his original procedures to their own liking and image to varying degrees of success. These providers wanted to get around Dr. Runels’ trademarks and skirt the licensing fees. Dr. Runels has rightly protected his trademarks to keep the value of his teaching and work intact. More important is the aggressive legal protections he has provided his students like me who pay the CMA membership fees so that our investments in his teaching remain of highest value.
I was so very proud to host Charles Runels, MD, and Alexandra Runnels, MD, at my former Laguna Beach office in February of 2020 right before the pandemic. We were able to share ideas and form a stronger relationship. It was a magical week.
Alex wanted to learn more of my office surgical techniques and how I ran my Cosmetic Gynecology practice. She had started a wonderful practice in San Antonio, Texas, that was top of the line in non-surgical treatments and she wanted more exposure to the way I did my surgeries.
Alex and I dove into several days of One-on-One surgery covering all areas of labiaplasty and vaginoplasty surgeries. She would combine this with the amazing personalized non-surgical training she received from Charles. Her San Antonio practice has become second to none for cosmetic gynecology for her region.
My staff and I took Charles and Alex to the iconic Las Brisas Restaurant in Laguna Beach and I just had to take this classic photo from outside. We decided way back then that we had to work together in some way to bring value to practices worldwide using our individual talents and skills and combining them to create an unmatched learning experience. Charles with CMA and me with Gynflix.
One of the first things Charles and I collaborated on was the creation of a special speculum for doing the O-Shot. And now the Clitoxin Shot. With my background of working with medical device companies, I was able to design and manufacture a half speculum with wings that pushed the labia minora out of the way to give a great view of the anterior compartment and injections site for the O-Shot. Charles’s input was important and I used his suggestions before the Alinsod O-Shot Speculum was released. You can purchase it at Marina Medical.
Now every spring in Fort Lauderdale we are able to meet and catch up. We typically present our newest research and findings and procedures at this Grand Daddy Cosmetic Gynecology Congress that the entire world goes to.
Many societies are honored to have an association with Dr. Runels. Here he is being welcomed by the leadership of SOLAGEF, the Latin American Society for Functional and Aesthetic Gynecology. This non-gynecologist has more memberships in gynecologic organizations than anyone I know. And he deserves all of those honorary memberships!
In December 2022, when the world was finally emerging from its pandemic cocoon, I flew to Fairhope, Alabama, and attended the Master Mind class Charles and Alex were sharing with a select group of CMA members. This was not about. how to do an O-Shot or P-Shot. This was how to think different and how to use the best tools available to get there.
Suzette Peterson, my webmaster of 20 years, and I flew in from Southern California. In this course I learned the secrets of how Charles built his CMA Empire and the tools he used to achieve those lofty goals he set. Two days for full-on no-filter sharing. Probably the most important impetus for me to start creating my Gynflix website to educate online. Charles shared his vision and for that I am eternally grateful. He encouraged me tremendously.

In 2023, Charles started teasing us of his work with PRP plus Botox injected into the clitoris. I was fascinated. I knew Charles and Alex were hot on the heels of something news and exciting. I would have to wait another year to hear of their work’s progress and results.
Charles has a way of captivating an audience. Everyone who is anyone in Cosmetic Gynecology are at the edge of their seats, taking pictures of the slides, recording his words, and mobbing him at the end of his talks. He is kind and patient and will talk to you and befriend you.
Charles Runels is humble in his approach to life and his acceptance of honors bestowed to him are graceful and heartwarming.
Charle’s work speaks for itself. He truly deserves accolades for his transformative contributions to Cosmetic Gynecology. He just does not win awards for best results for this and that. He does not just play with medical toys. He wins due to innovations and original research. He is not a copier but goes out on a limb to think of original processes and clinical applications that make a difference.
Clitoxin is the most recent original contribution Charles has shared with the world. He released the research he and Alex performed at ISCG 2024 this past March. His combination of PRP with Botulinum Toxin for genital use and improvement of sensitivity and sexual response has been groundbreaking and just getting known to the world. Who would have thought toxins could improve sexual response?!
